• If you are joining Auskick you need to complete the Auskick Application form & look for 'DONVALE MAGPIES'
• A DMJFC registration form and parent consent form will also need to be filled out 

Junior Football

• If you are playing football in any age group you need to complete the DMJFC registration form & also have your parent/guardian sign the parental consent form.
• If you have transferred from another club you need to complete an EFL rego and transfer form as well as the DMJFC registration & the parental consent forms.
• If you have never played with the EFL (Eastern Football League) you need to complete an EFL rego and transfer form (this includes all first year players) as well as the DMJFC registration & the parental consent forms.

If you have a player with a medical condition, please also complete a Medical Histroy form

 All applications can be posted or hand delivered to the club.
